Service Offerings
Personal Support Services (PSS):
We provide habilitative services and overnight support assistance to participants. This includes:
1. In-home skill development
2. Community integration and engagement skill development &
3. Overnight supports
Community Development Services (CDS):
With CDS we provide individuals with development and maintenance of skills related to community membership through engagement in community-based activities.
Community Living Group Home (CLGH):
We provide individuals with coordination, training, hands-on support and supervision for development, acquisition and maintenance of skills related to activities of daily living, instrumental activities of daily living, vocation and socialization.
Supported Living:
We provide participants with a variety of services to support living independently in the community. With this service participants learn self-direction and problem solving related to maintaining activities of daily living and instrumental activities of daily living required for the participant to live independently.
Employment Services:
This includes Discovery, Job Development, Follow-Along Supports, Ongoing Job Support, Self-employment Development Support, and Co-worker Employment Support.
Day Habilitation Services:
This service provides individuals with development and maintenance of skills related to activities daily living, development of skills related to learning new skills for employment, socially acceptable behavior etc
Nursing Support Services (NSS):
This service provides individuals with nursing assessments, perform nursing consultation and nursing delegation services
Respite Care Service:
This service provides relief for parents, guardians and family caregivers. This includes Direct support services, medication administration, implementation of behavioral support plan and overnight support.
Transportation Services:
This service assists the participants in the ability to independently access community activities within their own community. This includes travel training, transportation services provided by different modalities, including but not limited to public and community transportation, taxi services, and non-traditional transportation providers like ride-sharing services etc.
